We respect the confidentiality of your private information.

Private information is protected under Canada’s Freedom of Information and Protection of Privacy Act. We will not provide any information about you or your treatment at the Nanaimo Pain Clinic to anyone outside the clinic, without your signed approval via a Release of Information Form.

Exceptions would only occur:

• If we have concerns regarding harm to yourself or others;

• If we are subpoenaed to testify in court proceedings

In either of these circumstances, we will make every reasonable attempt to obtain your prior written consent.

We respect the privacy of all patients of the Nanaimo Pain Clinic, and ask that you do as well. Please keep confidential anything you may hear or see while at The Nanaimo Pain Clinic.
